Typically, the admission process requires the following procedures:

After you have reviewed the Institute program description and would like more information regarding admission, please call (303)-238-1231 or contact us at info@aficonline.com.
Contact the Insurance Coordinator concerning financial matters at (303)-238-1231.
After you have contacted the Institute directly and have been instructed to do so, download the Family System Intake Document and complete the document in its entirety. (The file is in Microsoft Word format. To download, please right-mouse-click on the link above and select "Save Target As", then select a destination on your hard drive to save the file to.)

Because of confidentiality concerns, we ask that you please print the Intake Document, read the instructions, fill out the form completely and mail or fax the requested pages to:

If applicable, request previous treatment records, discharge summaries or answers to the 4 questions as referenced on page 2 of the Family System Intake Document.
Attend the Parents Survival and Education Group on Tuesday evenings, 6:00-7:30 pm, complete the parents’ feedback sheet after the meeting and leave it with the Discussion Leader. (Call first to assure that the meeting will be held on that night)
Request a date and time for an initial interview/evaluation.
