Admission

Typically, the admission process requires the following procedures:

1. After you have reviewed the Institute description and would like more information regarding admission, call us at (303)-238-1231 or contact us at info@aficonline.com.
2. Contact the Insurance Coordinator concerning financial matters at (303)-238-1231.
3. After you have contacted the Institute directly and have been instructed to do so, Download the Family System Intake Document and complete it in its entirety. (The file is in Microsoft Word format. To download, please right-mouse-click on the link above and select "Save Target As", then select a destination on your hard drive to save the file to.)
   
   Because of confidentiality concerns, we ask that you please print and fill out the file, then mail or fax to:

4. Request previous records or discharge summaries or answers to the 4 questions as referenced on page 2 of the Family System Intake Document.
5. Attend the Parents Survival and Education Group on Tuesday evenings, 6:00-7:30 pm, complete the parents feedback sheet after the meeting and leave it with the Discussion Leader.
6. Request a date for an initial interview/evaluation.
